MEMBERS BENEFITS
Primary Care Partners Northwest offers a variety of benefits to our members allowing us to deliver more personalized care. Our patients receive:
- Priority and expanded appointments, including telephone visits;
- Coordination with consultants who are providing hospital or other outpatient care, this can include telephone conference calls during consultations;
- Based on an annual, comprehensive physical examination and other indicated investigations, members will receive annual health reviews and report;
- Members will receive an initial and annual Health Risk Assessment and Score using relevant medical, behavioral and dietary information, including cancer and cardiac risk factors which will include a plan for treatment and risk reduction;
- Members wishing to have their medical record maintained in a portable, electronic form will be provided an encrypted USB thumb drive containing medical history, laboratory results, cardiogram and other relevant medical information;
- A web accessible secure electronic medical record will be developed upon request;
- Health enhancement discussions will be offered to members quarterly. Members will also receive a quarterly newsletter covering relevant healthcare topics;
- Members receive an individual membership to the Cardiac Fitness Center at the McMurray Medical Office Building located on the main Hospital campus;
- Tailored travel healthcare advice;
- Members receive complimentary access to a variety of health education opportunities designed for lay audiences offered through Northwest Hospital’s Community Health Education & Simulation Center;
- Complimentary parking at any Northwest Hospital location;
- Associate provider coverage during a principal provider’s absence. This includes early member notification of a principal provider’s planned absence, including instruction on how to contact the associate provider or other covering physician.
